axsPolicy

Verify

Coverage intelligence, verified. Formulary data tells you what the policy says. axsPolicy tells you what actually happens — by layering claims-observed signals on top of stated coverage to complete the picture.

The Gap

Stated policy is only half the picture

Traditional coverage data — formulary status, tier, PA/ST requirements — tells you what the policy is supposed to be. But it doesn't tell you what's actually happening at the pharmacy counter.

Claims get rejected for reasons not in the formulary file. Reversals signal coverage changes that haven't been updated. Patients stall for weeks waiting on PAs that should take days. Copay accumulators and maximizer programs silently erode your support investment.

axsPolicy closes this gap by layering pharmacy and medical claims signals on top of stated policy data — so you see coverage as it actually operates, not just as it's documented.

Claims-Observed Signals

The intelligence that stated policy doesn't capture

axsPolicy uses pharmacy and medical claims data to surface coverage behaviors that traditional policy files miss entirely.

Rejections
Claims rejection rates by plan, drug, and reason code. Patterns that reveal unstated coverage restrictions, benefit exclusions, and enforcement gaps.
Claims signal
Reversals
Claims that were approved then reversed — signaling mid-cycle coverage changes, retroactive policy updates, or benefit reprocessing that stated data doesn't reflect.
Claims signal
Stalled Coverage
Time-to-fill metrics that expose how long patients actually wait. When stated PA turnaround is 72 hours but observed fill times are 14+ days, the policy isn't the problem — the process is.
Time signal
Maximizers
Detection of copay maximizer programs that redirect manufacturer support to plan benefit — eroding your copay card investment and shifting cost to the patient at accumulator reset.
Financial signal
Accumulators
Copay accumulator programs that prevent manufacturer assistance from counting toward patient deductibles and OOP maximums. Rarely in stated policy — always visible in claims.
Financial signal
SP Network Routing
Which specialty pharmacies actually dispense for a given plan — not which ones are theoretically in-network. Reveals exclusive SP contracts and narrow networks that stated policy often omits.

Capabilities

Stated + observed = complete coverage intelligence

axsPolicy doesn't replace formulary data — it completes it. By combining traditional policy files with claims-observed signals, you get a coverage picture you can actually trust.

Formulary Verification
Stated tier, PA, step therapy, and quantity limits — verified against claims data. When the formulary says one thing and claims say another, you'll know.
Claims-Observed Layer
Rejection rates, reversal patterns, stalled coverage timing, and SP routing — derived from pharmacy and medical claims to surface coverage behaviors that policy files miss.
Accumulator / Maximizer
Detects copay accumulator and maximizer programs across plans — information almost never in stated policy data but directly visible in claims payment patterns.
Lives Integration
Coverage data tied to plan-level lives counts — so you weight your access landscape by actual patient volume, not just formulary wins on paper.
SP Network Intelligence
Actual specialty pharmacy dispensing patterns by plan — revealing exclusive contracts, narrow networks, and routing behavior that stated network files don't capture.
axsID Connected
Every coverage record is linked to an axsID payer identity — positioned in the 7-level ontology. Coverage intelligence inherits the same plan, payer, and channel structure.
Who it's for

Coverage intelligence for teams that need to act, not just report

Market Access
Verify that formulary wins actually deliver
You won the formulary. But are patients actually getting filled? axsPolicy cross-references stated coverage with observed fill rates, rejection patterns, and time-to-therapy — so you can prove real-world access, not just contractual wins.
Hub Operations
Anticipate access barriers before intake
Know ahead of time which plans have high rejection rates, active accumulator programs, or exclusive SP routing. Route patients to the right support path on day one instead of reacting after the claim fails.
Contracting & Trade
Expose coverage erosion in rebate negotiations
When stated coverage is "preferred tier" but observed data shows rising rejections and maximizer adoption, you have the evidence to renegotiate. axsPolicy gives contracting teams claims-verified leverage.
Patient Services
Detect accumulator and maximizer risk
Copay card budgets are finite. When accumulators prevent your support from counting toward patient OOP, and maximizers redirect it to plan benefit, you need to know which plans are doing this — before renewal season.
Coverage Depth

Stated + observed

2
Coverage Layers
Stated policy from formulary files and observed coverage from pharmacy and medical claims — combined in a single, verified view.
6
Observed Signal Types
Rejections, reversals, stalled coverage, maximizers, accumulators, and SP network routing — derived from claims data.
7
Ontology Levels
Every coverage record inherits its axsID position — channel, PBM, payer, plan, GPO, formulary, and contract level.
